Trelegy Ellipta 200/62.5/25 mcg is an inhaler that combines three active ingredients from three distinct drug classes: an inhaled corticosteroid (fluticasone furoate), a long-acting muscarinic antagonist (umeclidinium), and a long-acting beta2-adrenergic agonist (vilanterol). Delivered via a single inhalation device, it is used as a long-term maintenance treatment for asthma in adults by relaxing airway smooth muscle and improving airflow.

Trelegy Ellipta how to use

  • Use one inhalation once daily at approximately the same time every day.

  • Do not use more than one inhalation in a 24-hour period.

  • Use the device strictly according to the inhalation steps detailed in the package leaflet.

  • Open the cover of the Ellipta inhaler only when ready to take a dose; opening the cover loads one dose.

  • Do not shake the inhaler.

  • Exhale fully away from the device, then place the mouthpiece between your lips and close them firmly around it.

  • Take a long, deep, steady breath in through your mouth. Remove the inhaler and hold your breath for a few seconds or for as long as comfortable.

  • Close the inhaler cover after use.

  • After inhalation, rinse your mouth with water and spit it out (do not swallow) to help reduce the risk of oral fungal infections (thrush).

  • Do not use Trelegy Ellipta to relieve acute asthma attacks. Always use a short-acting fast-relief rescue inhaler prescribed by your physician when needed.

Precautions and Warnings

  • Indicated for adult patients with asthma as prescribed by a physician.

  • Not a quick-relief medication and must not be used to treat acute bronchospasm or sudden asthma attacks.

  • Do not use more than once daily.

  • Do not use concurrently with other medications containing long-acting beta2-agonists (LABAs) or long-acting muscarinic antagonists (LAMAs) unless directed by a physician.

  • Consult a physician prior to use in cases of underlying cardiovascular disease, hypertension, or cardiac arrhythmias.

  • Use with caution in patients with narrow-angle glaucoma, prostatic hyperplasia, or urinary retention, as umeclidinium may exacerbate these conditions.

  • Inform your physician if you have severe hepatic impairment.

  • Inhaled corticosteroids may increase the risk of fungal infections of the mouth and throat; rinsing the mouth with water after each dose is strongly recommended.

  • Inform your doctor immediately if experiencing symptoms such as eye pain, blurred vision, or acute worsening of breathing difficulties right after inhalation.

  • Do not discontinue treatment or alter the dosage without consulting your physician.

Side Effects

Possible side effects may include:

  • Headache

  • Sore throat or throat irritation

  • Hoarseness (dysphonia)

  • Cough

  • Sinusitis

  • Upper respiratory tract infections

  • Dry mouth

  • Constipation

  • Dysgeusia (altered sense of taste)

  • Musculoskeletal or joint pain in some patients

Fungal infections of the mouth or throat (oral candidiasis) may occur; rinsing the mouth with water after use helps prevent this.

Less common or more serious adverse events include increased heart rate, cardiac arrhythmias, elevated intraocular pressure, or urinary retention.

In rare instances, paradoxical bronchospasm may occur immediately after inhalation, causing a sudden increase in wheezing and shortness of breath. If this occurs, discontinue Trelegy immediately, use your prescribed rescue medication, and seek medical attention.

Drug Interactions

The efficacy of Trelegy or the risk of its adverse effects may be affected when co-administered with certain drugs, including:

  • Beta-blockers: May diminish the bronchodilating effect of vilanterol or induce severe bronchospasm in asthma patients.

  • Strong CYP3A4 inhibitors (e.g., ketoconazole): May increase systemic exposure to fluticasone furoate and vilanterol.

  • Non-potassium-sparing diuretics: Concurrent use with beta2-agonists may increase the risk of hypokalemia.

  • Tricyclic antidepressants, MAO inhibitors, and QT-prolonging drugs: May potentiate the cardiovascular effects of vilanterol.

  • Other anticholinergic agents: Concurrent use with umeclidinium may increase systemic anticholinergic side effects.

  • Inform your doctor or pharmacist of all medications, over-the-counter products, and supplements you are using prior to initiating therapy.

Storage Conditions

  • Keep out of reach of children.

  • Store at room temperature between 20°C–25°C (excursions permitted between 15°C–30°C).

  • Keep the inhaler inside its original tray packaging until ready for initial use.

  • The package contains a desiccant packet to protect the medicine from moisture; do not open, eat, or inhale the desiccant.

  • Upon opening the foil tray, write the opening date on the inhaler label.

  • Use the device within the timeframe specified in the package insert after opening.

  • Do not store in damp places or near heat sources.

  • Do not use after the expiration date printed on the packaging.

  • The dose counter starts at 30 and counts down by 1 each time the cover is opened and a dose is prepared.
